Key Responsibilities:

  • Collaborate with an interdisciplinary team to develop and manage patient care plans
  • Provide education and support to patients and families to facilitate understanding of treatment plans
  • Act as a liaison between patients, families, healthcare teams, and community resources to coordinate care
  • Monitor patient progress and adjust care plans as needed

Requirements:

  • Associate's degree in Nursing required; Bachelor's degree preferred

  • Professional certification in clinical area required within one year of hire
