Position Summary

The Community Health PrEP Navigator plays a crucial role in offering support, education, and guidance to clients at elevated risk for HIV, aiming to enhance access and adherence to PrEP (Pre-exposure Prophylaxis) to avert HIV infection. This individual will engage with clients through phone calls and in-person visits to encourage participation in services. The PrEP Navigator will deliver education on HIV, STDs, and general health and wellness in both individual and group settings, focusing outreach efforts on specific populations as determined by programmatic requirements.

The Community Health PrEP Navigator will report directly to the Senior Director of Community Health and Wellness and will collaborate with various departments including Medical, Medical Adherence, Behavioral Health, Public Benefits, and Insurance Navigation, among others. Participation in Whitman-Walker Health activities and training sessions is expected, following the directives established by the organization.

Role-Specific Responsibilities:

  • Conduct community outreach to collaborate with other agency navigators to identify and support activities that engage new clients in PrEP services.
  • Facilitate educational sessions with targeted populations, providing information on HIV, STIs, and general health and wellness using culturally sensitive materials and approaches.
  • Inform patients referred for PrEP about available services and assist in scheduling PrEP appointments for those interested and potentially eligible.
  • Provide in-person support for initial PrEP appointments.
  • Collaborate with the Director of Community Health and Quality Improvement and the Informatics Department to develop and implement data collection protocols and referral processes for high-risk HIV-negative individuals.
  • Administer HIV and STD testing for clients in both clinic and mobile-testing environments, offering prevention counseling and referring clients to appropriate programs and providers.
  • Assist new clients in overcoming barriers to accessing PrEP and related support services.
  • Educate patients on self-advocacy skills, empowering them to communicate effectively with their healthcare teams.
  • Document all activities in accordance with established standards, collecting data for production in reports and assisting in their creation as needed.
  • Support the maintenance of testing kits, incentive items, and other physical inventory.
  • Participate in meetings with stakeholders and assist with monthly reporting.
  • Work with patients referred from Clinical Testing and Referral Services (CTRS) to help engage and retain them in PrEP care.
  • Assist potential patients with initial insurance navigation and referrals to the Public Benefits and Insurance Navigation department.
  • Help patients prescribed Truvada for PrEP obtain financial assistance through various channels.
  • Coordinate with medical providers to assist patients in need of financial support on an on-call basis.
  • Collaborate with the pharmacy to troubleshoot and resolve patient issues as necessary.
  • Support patients referred from the PrEP Clinic who are facing adherence challenges.
  • Conduct follow-ups to assist patients in maintaining adherence to PrEP through routine check-ins.
  • Schedule follow-up appointments for patients and provide adherence support.
  • Perform additional duties as assigned, including special tasks related to emergency events.

Role-Specific Knowledge, Skills, and Abilities:

  • Knowledge of ambulatory healthcare, PEP/PrEP, HIV, and STIs.
  • Familiarity with the local community.
  • Experience working with diverse populations, including MSM of color and transgender individuals.
  • Understanding of HIV/STD risk factors and counseling techniques.
  • Ability to conduct sensitive and empathetic interviews.
  • Strong interpersonal skills for educating and counseling clients from diverse backgrounds.
  • Excellent communication skills, with the ability to engage groups and individuals effectively.
  • Analytical skills for interpreting and presenting data related to community health.
  • Organizational skills to manage multiple tasks and meet deadlines.

Education and Experience Required:

  • Bachelor's Degree or at least four years of relevant experience.
  • Fluency in Spanish or Amharic required.
  • HIV testing and training certification preferred; experience providing counseling to individuals living with HIV preferred.
  • Experience working with diverse populations and the ability to collaborate effectively with various groups.
  • Completion of phlebotomy training is strongly preferred.
