Position Overview

The Patient Care Navigator (PCN) is an essential member of the team at Mary's Center, serving as the initial point of contact for patients and ensuring a welcoming environment. Bilingual candidates are highly encouraged to apply.

Reporting Structure

The Patient Care Navigator will report directly to the Patient Care Navigator Manager at their designated location.

Key Responsibilities

To excel in this role, the individual must effectively carry out the following duties:
  • Welcome patients, visitors, and others to Mary's Center with a friendly demeanor.
  • Foster a positive and efficient office atmosphere by demonstrating professional behavior.
  • Relay important information to patients and the clinic team.
  • Comply with all confidentiality and HIPAA regulations set by Mary's Center.
  • Collaborate with the clinical team to enhance quality and health outcomes.
  • Coordinate patient care and services effectively.
  • Work with external healthcare providers, specialists, and hospital services to ensure comprehensive care.
  • Assist in enrolling patients into the care team.
  • Gather socio-demographic data for population health management purposes.
  • Engage in patient education initiatives and projects.
  • Collect and monitor data necessary for patient care and health outcomes reporting.
  • Implement clinical questionnaires and surveys to prepare for patient care.
  • Evaluate care affordability and assist with enrollment in insurance or discount programs.
  • Organize and maintain patient records for clinical care and public health purposes.
    • Transfer public health information to State health departments, including reportable infections and vaccinations.
    • Share clinical care information with other providers through the regional health information exchange.
  • Prepare and maintain the patient care area to ensure a conducive environment.
  • Perform additional duties as assigned by supervisors.
Qualifications

The following qualifications are necessary for this position:

Education and Experience - A minimum of a High School Diploma or GED equivalent is required. Previous experience in a medical office setting as a receptionist is preferred.

Physical Requirements - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk, or hear; must be able to lift objects weighing up to twenty-five (25) pounds.

Work Environment - Primarily in a standard front office setting with a quiet to moderate noise level. Exposure to extreme weather conditions may occur.
