Compensation:
$24/hour

KEY RESPONSIBILITIES
Plan, develop, organize, and oversee the Social Services Program.
Coordinate and engage in family and patient activities aimed at enhancing social interaction, reality orientation, and cognitive stimulation.

Provide counseling to patients and families, addressing feelings related to death or dying, as well as other emotional, mental, environmental, or physical challenges.

Complete the social service section of the MDS within 14 days of patient admission.
Manage admissions and discharges.
Act as a liaison with social, health, and community organizations.
Facilitate Patient and Family Councils as necessary.
Assist in resolving grievances when requested.
Understand the necessary documentation for Advance Directives.

Enhance the reputation of the organization by demonstrating servant leadership and maintaining open lines of communication.

Contribute to committees, workgroups, project management, and other collaborative initiatives within the organization.
Perform additional duties as required to ensure the success of the organization.

SKILLS AND QUALIFICATIONS
Proficient in computer usage, particularly Microsoft Office, Excel, and PowerPoint.
Strong organizational and prioritization skills are essential.
Excellent verbal and written communication skills.
Ability to engage effectively with colleagues, patients, family members, and visitors, providing a warm and friendly greeting and an approachable demeanor. Respond to concerns with a helpful, caring attitude, and answer questions professionally when appropriate.

MINIMUM REQUIREMENTS

A Bachelor’s degree in social work or a minimum of two years of supervisory experience in a hospital, skilled nursing facility, or other medical settings is preferred.


SUPERVISORY RESPONSIBILITIES
Supervision of social services assistants, if necessary.

LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.

MATHEMATICAL SKILLS
Ability to perform basic arithmetic operations using whole numbers, common fractions, and decimals.


REASONING ABILITY
Ability to apply common sense understanding to carry out instructions provided in written, oral, or diagram format. Ability to address problems involving several concrete variables in standardized situations.

COMPUTER APPLICATION SKILLS
This position requires proficiency with the following software applications:
Microsoft Outlook, Microsoft Excel, Microsoft Word.
