About Your Role

As a key member of our team, you will play a vital role in supporting the aging adults and adults with disabling conditions by providing case management, coordinated care, caregiver support, and access to the Long Term Services and Supports (LTSS) through a collaborative partnership with the RI Department of Human Services (RIDHS), the Office of Healthy Aging (OHA), and the Executive Office of Health and Human Services (EOHHS).

Key Responsibilities
  • Manage referrals for services both in-house and community-based programs, ensuring timely and efficient processing.
  • Develop and implement a systematic approach to referral management, streamlining processes and improving communication.
  • Collaborate with clients and outside organizations to ensure fulfillment of requests and provide excellent customer service.
  • Monitor daily census and data collection for all programs, maintaining accurate records and reporting.
  • Perform data entry for billing and work with multiple agencies and platforms to ensure seamless operations.
Requirements and Qualifications
  • High school diploma or equivalent with experience in a related field; associate degree preferred.
  • Minimum 1 year experience in a medical setting, with a strong understanding of case management and referral processes.
  • Bilingual in Spanish preferred, with excellent communication and interpersonal skills.
  • Strong organizational, critical thinking, and time-management skills, with the ability to work in a fast-paced environment.
  • Basic computer skills, with knowledge of Microsoft Office 365 and ability to learn new software.
Physical and Travel Requirements

This role requires time sitting, standing, walking, driving, carrying, and lifting up to 25lbs, as well as operating desktop, laptop, and/or cell phone, and communicating with colleagues. Statewide travel is required, with site location in Middletown.
