Lead Case Manager

Mission: To enhance member health through an integrated managed care system. Our Enhanced Care Management Program or (ECM) provides care coordination and management to our clients with complex needs, so that they may receive the vital services they need in order to stay healthy.
Position Overview:
The ECM Lead Care Manager will manage community outreach & engagement, eligibility assessments, and enrollment for our Enhanced Care Management (ECM) program. This role involves engaging members, creating and implementing care plans, and providing ongoing support.
Key Responsibilities:
- Engagement & Enrollment: Connect with potential ECM members, assess eligibility, and enroll individuals into the program.
- Care Planning: Conduct risk assessments, develop and implement patient-centered care plans, and update plans quarterly.
- Patient Support: Educate members on self-management, support behavior change, and coordinate with social services and healthcare providers.
- Documentation: Maintain accurate records in the Electronic Health Record (EHR) and track care management activities.
Qualifications:
- Mid-level experience in care management or related fields.
- Strong skills in communication, empathy, and critical thinking.
- Ability to handle multiple tasks and work independently.
- Experience with motivational interviewing and trauma-informed care is a plus.
- Comfort with both in-person and remote patient interactions.
Reports to: ECM Program Manager
Other Details:
- Open to seeing patients in their preferred locations.
- Regular in-person meetings with patients are required.
