Who We Are

Catholic Community Services Counseling, Recovery & Wellness (CReW) Program is a licensed community mental health and substance use disorder treatment program and provider of the King County Integrated Care Network (KC-ICN). Our collaborative team focuses on serving adults with mental illness and/or substance use disorders to help improve the quality of life for our clients and our great community. Services offered utilize Harm Reduction and Recovery principles, with an emphasis on building collaborative relationships with the persons served as the first step towards lasting engagement in treatment. Range of services includes diagnostic assessments, individual therapy, peer support, case management, group counseling, skills training, care coordination, medication management, and psychoeducation. Our team was created out of the need for onsite supportive services in our agency housing program and we adapt our services to best fit the unique needs of our clients in a variety of locations within South King County. Position Summary

The Intake Specialist is one of the primary points of contact for the CReW program and is responsible for completing the behavioral health intake process with all new clients, as prescribed by Medicaid and WAC. This assessment includes: obtaining informed consent, conducting a comprehensive bio-psycho-social assessment, gathering demographic and medical history details, determining an initial diagnostic formulation using clinical and diagnostic assessment tools, and determining initial treatment areas for community behavioral health services. Since programming offers integrated mental health and substance use treatment services, the Intake Specialist will complete one assessment that meets state requirements for both scopes of practice, while providing support to the client throughout the clinical intake process. This position will also be responsible for making referrals to higher level of care settings as appropriate based on unique needs, including referrals for substance use detox and intensive inpatient facilities. Work is performed under the supervision of the Clinical Supervisor or Site Director, including close collaboration to assist with caseload assignments to ensure that clients receive timely and appropriate follow up support to initiate treatment.

Responsibilities:

Clinical Intakes

  • Utilize Behavioral Health clinical assessment and engagement skills to meet with clients to gather demographic, clinical/situation information
  • Perform initial assessment and clinical diagnosis based on assessed service need and intensity.
  • As part of the clinical intake process, complete behavioral health clinical assessment tools including the PHQ-9, PCL, GAD, ASAM, CALOCUS, GAINS SS and Quadrant determination.
  • Demonstrate familiarity with DSM criteria for clinical mental health diagnoses and formulate a clinical diagnosis based on client background and presenting behavioral health issues and concerns.
  • Request Releases of Information from the client in other to gather medical and behavioral health histories from previous providers.
  • Engage the client during the clinical intake session, assessing for immediate safety needs and concerns, gather and clinically summarize client history and presenting issues prioritizing urgent safety considerations in the intake process.
  • Complete all Medicaid and WAC required intake documentation and client enrollment information, ensuring appropriate Medicaid coding and entering information into an electronic health record as required by funder and WAC timeframes.
  • Complete documentation required by funders to refer and obtain prior authorization, as needed, for intensive inpatient substance use treatment facilities. Coordinate with funders and inpatient treatment facilities to obtain treatment bed dates and relay necessary information to coordinate successful arrival on scheduled dates.
  • Maintain on-going clinical contact with clients who meet criteria for services, pending assignment to an ongoing primary clinician, ensure appropriate and timely coordination, collaboration and support for behavioral health supports.
  • Demonstrate flexibility and organization to respond to urgent referral requests in the community as needed, including flexibility to meet with clients in their permanent supportive housing programs or shelters if needed to build trust and connection.
  • Maintain knowledge of relevant social and behavioral health services to ensure all new clients are given information about programs and supports to meet basic needs (i.e. Foundational Community Supports, shelter or housing providers Regional Access Points for Coordinated Entry, Domestic Violence programs, etc)
  • Complete complex clinical intake documentation as required by funders and Medicaid, tracking client enrollments and waitlists, as needed to monitor program growth or expansion.
  • Utilize tracking processes to monitor timeliness of intake response/process and case assignment to on-going clinician.
  • Support access to services in the community: represent CReW in the community by community outreach and collaboration. Develop and maintain strong and active relationships with funders, Behavioral health and Medical providers, permanent supportive housing programs, shelters, and other referral sources.
  • Carry cases as primary or assisting clinician as assigned
  • Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).

Utilization Tracking

  • Maintain tracking list of process steps for completion of required referral and intake activities, such as clinical assessment completion, referral dispositions, scheduled and completed behavioral health intakes, turn-aways, walk-outs, and no-shows.

Quality Improvement

  • Participate in audit preparation and review of clinical records to ensure content, format, timeliness and quality standards of intake and initial documentation are Medicaid compliant for all clients.

Other duties as assigned

Qualifications:
  • Masters degree in social service field with two or more years of clinical experience working with children and families including knowledge of behavioral health and child welfare service systems.
  • Mental Health Professional or eligibility, per WAC requirement.
  • Substance Use Disorder Professional or Trainee (SUDP/T) credential from WA State Department of Health (formerly CDP/T).
  • Agency Affiliated Counselor Registration from WA DOH, or higher credential, eligible if not active at time of hire
  • Clinical knowledge of DSM diagnostic and clinical criteria as well as the ASAM Patient Placement Criteria.
  • Demonstrated clinical engagement skills which are influenced by principles of Motivational Interviewing, Harm Reduction, and Trauma Informed Care.
  • Computer proficiency in Excel and Word.
  • Ability to quickly learn and master a Behavioral Health (EHR) electronic health record systems.
  • Well-developed office skills, including phone systems, data entry, etc.
