Responsible for assisting DuPage Health Coalition clients to access health promoting community-based services, including dental services and fitness/wellness services.

**Essential Duties**:

- Coordinate process of accessing available dental services for Access DuPage patients, including processing requests, contacting providers to seek care or records, communicating with and educating patients, ensuring smooth registration and scheduling, and other elements as needed.
- Collaborate effectively and efficiently with team members from DuPage Health Coalition and community partners, including oral health providers and fitness and wellness service sites.
- Enter data and maintain accurate database for tracking utilization of services provided and coordinated; Produce or assist in production of reports regarding service utilization and other ad hoc requests.
- Provide clients, providers, and facilities documentation of coordinated services.
- Participate in marketing and outreach opportunities to increase program visibility, increase program and provider participation, increase community awareness, and increase client understanding of all programs, with special focus on growing capacity and success of oral health and Access Fit programming.
- Become sufficiently knowledgeable regarding DuPage County health and community resources to support members and partners needing services outside DHC scope.

Provide coverage for teammates as requested and needed.
- Perform other duties as requested.

**Qualifications**:

- Bilingual: Spanish/English required.
- Knowledge, skills, and abilities required: Must have the ability to work independently, have strong communication skills, and experience with computers. Ability to manage multiple tasks simultaneously. Customer service skills a must. A service-oriented personality is required.

**Supervisory Responsibility**:
None

**Work Environment**:
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

**Physical Demands**:
This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand as necessary.

**Position Type and Expected Hours of Work**:
This is a full-time hybrid position. Typical days and hours of work are contained within the hours of Monday through Friday, 8:00 a.m. to 4 p.m., but hours may vary at the request of leadership.

**Travel**:
Travel to outreach/community events. No overnight stays. Mileage reimbursement is offered.
