Position Description:
Works effectively with diverse families by providing client-centered services to support program entry and exit of program participants, and supports the program supervisor with data management for Goodwill of Central and Northern Arizona (GCNA) and its affiliated entities.

Essential Duties and Responsibilities:

- Reviews the program occupancy report and exit schedule daily. Anticipates the number of new intakes that need to be completed to maintain occupancy level.
- Initiates contact with referrals provided. Conducts phone screenings for eligibility. Matches location need to location availability, conducts intake, and assigns to an available case manager.
- Conducts assessments and submits requests for financial assistance for food and transportation.
- Supports coordination of program exit. Conducts post-exit assessments and updates reports.
- Updates Homeless Management Information System (HMIS) daily. Ensures the confidentiality and safety of case files and client information.
- Maintains and submits mileage logs, client expenses, and other required documentation accurately and in a timely fashion.
- Maintains and enhances resource directories, and distributes materials.
- Studies services available internally and through existing partnerships and their eligibility requirements.
- Analyzes the types, frequency, and other characteristics of services that program participants use, need, and want. Documents findings in detail.
- Establishes links to community agencies and service providers.
- Attends staff meetings, supervision meetings, community meetings, and training workshops as needed.
- May maintain a small caseload of less than 5 families, as assigned.
- Assists program participants in developing an individual case management plan, including financial planning, and housing goals.
- Provides weekly contact with assigned families, which includes home/community visits, and conducts weekly room inspections.
- Completes and submits weekly requests for financial assistance in food, transportation, and other miscellaneous items.
- Provides support in benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their stabilization goals. Monitors the ongoing provision of services from outside agencies.
- Responds to crisis situations as needed. Provides crisis intervention services focused on enhancing the program participant’s ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate their care.
- Ensures case files are up to date and ensures that all necessary client information is entered into HMIS in a timely manner. Ensures the confidentiality and safety of case files and client information.

Minimum Qualifications (Education, Experience, Skills):

- One year of experience with people experiencing homelessness.
- High School Diploma/GED required and three years of human services experience, or A.A. Degree and one year of human services experience.
- Sensitivity to socioeconomic and cultural characteristics of the populations served.
- Creative thinking, adaptive personality, and a conviction about the capacity of people to grow and change.
- Awareness and commitment to Housing First and coordinated entry procedures, and other housing-based strategies for ending homelessness.
- Commitment to maintain a role that empowers program participants and intervenes appropriately to meet service goals.
- Exceptional skills in customer service to facilitate positive ongoing relationships between program participants, service providers, housing teams, and the community.
- Interpersonal communication skills that allow for professional and positive relationships with co-workers, participants, families, and other community members, evidenced by a non-judgmental approach, effective listening, and positive interactions.
- Level 1 Fingerprint Clearance Card.
- Reliable transportation, Valid driver’s license and auto insurance.
- Ability to travel county-wide and work a flexible schedule if needed.
- Ability to pass a background check and drug screen.
- Ability to speak and read English proficiently, fluency in Spanish is preferred.
