**What we are looking for**:
We are seeking an experienced and highly motivated Lead Care Manager Site Supervisor to oversee our team of Care Managers at our residential care facility. In this vital leadership role, you will be responsible for managing the daily operations and ensuring the highest quality of care and support for our residents.

**Responsibilities**:

- **Supervision**: Supervise and provide guidance to a team of Care Managers, ensuring they adhere to established policies, procedures, and care standards.
- **Communication**: Conduct regular staff meetings, provide training, and support the professional development of the Care Manager team.
- **Management**: Monitor and ensure the accurate and timely documentation of resident care plans, progress notes, and incident reports.
- **Collaboration**: Collaborate with the interdisciplinary team, including clinicians, social workers, and other healthcare professionals, to develop and implement individualized care plans for members.
- **Escalations**: Respond to and resolve resident and family concerns to any escalated challenges outside of the capabilities of the Lead Care Managers.
- **Compliance**: Ensure compliance with all applicable regulations, standards, and organizational policies related to resident care and safety.
- **Participation**: Participate in the recruitment, onboarding, and performance evaluation processes for Care Managers.
- **Development**:Work in collaboration with the upper management to build on the internal and external processes, systems, and workflows of Lead Care Managers.
- **Reporting**: Provide monthly and quarterly reports, based on staff performance and member management. While maintaining open communication with the facility management team and contribute to strategic planning and decision-making processes on a weekly basis.

Pay: $55,000.00 - $68,000.00 per year
