Nursing Coordinator

Responsibilities:
Initial Report/ Start of Care pulled up daily
Discharge Report to be run daily
Reassessment Report to be run weekly
Assigning RN to complete the visits
Following up with RN on the fist day of service to remind them about the visits and confirming it was completed
Following up on RN paperwork
Saving all documents in patient folder
POC to be entered in exchange
MD Order to be send out
HHA supervision to be send to HR
Completes Quality Survey JOT form
Mail POC to patient if needed
Help RN schedule visit if needed
Assist with authorization for visits as needed
Have knowledge and be able to use the software’s we are currently using
Assigning Incident visits to RN when it is received via smart sheets
Requirements:
High school degree or equivalent required.
Experience in healthcare/home care. - Preferred
Knowledge of HHA Exchange. - Preferred
Comfort with Microsoft Office especially excel and similar programs.
Ability to communicate effectively with employees, patients, and their families
Ability to define problems and tasks, collect data and establish facts, take action and resolve issues
