Job Title: Professional Nursing Care Coordinator

About the Role:

The Professional Nursing Care Coordinator at AdvantageCare Physicians will serve as a liaison between patients, staff, and physicians, supporting the Advanced Care Model.

Key Responsibilities:

  • Identify patients who may need point-of-care testing as well as patients with special needs, coordinating outpatient work-up and/or treatment plans when required.
  • Accountable for the administration, treatment, and training of patient self-administered drugs.
  • Serve as a clinical liaison between the call center and medical office, triaging urgent same-day or double-booked appointment requests.

Requirements:

  • Graduate of an accredited school of nursing (R)
  • Bachelor's Degree in Nursing (P)
  • ACLS required for GI and Cardiology
  • Work experience as a nurse in a hospital or ambulatory setting (P)

Salary:$60,000 - $75,000 per year
