Job Description
As a vital part of our interdisciplinary team, the Nurse Care Coordinator will provide expert care coordination, ensuring seamless transitions between healthcare settings and optimizing patient outcomes. This critical role demands attention to detail, strong organizational skills, and effective communication with various stakeholders.
Main Responsibilities
* Facilitate patient transitions between healthcare settings, guaranteeing continuity of care and minimizing disruptions
* Develop and implement personalized care plans, incorporating input from multiple healthcare disciplines
* Coordinate appointments, tests, and procedures, streamlining patient flow and reducing wait times
* Foster open communication among patients, family members, and healthcare providers to ensure informed decision-making
* Collaborate with multidisciplinary teams to identify areas for process improvements and implement changes that enhance patient care and satisfaction
Qualifications
* Bachelor's degree in Nursing or related field
* Active licensure as a Registered Nurse in the state of employment
* At least 3 years of experience in clinical nursing, case management, or a related field
* Proven ability to multitask, prioritize tasks, and manage competing demands
* Strong analytical and problem-solving skills, with a focus on patient-centered care
