Job Overview
Birch Family Services Inc. is seeking a qualified Nursing Care Coordinator to join our team. As a Nursing Care Coordinator, you will be responsible for providing medical services in response to health problems, promoting health and safety, and collaborating with clinical and ancillary staff.

About the Role
This position requires the review of various assessments, logs, etc., and follow-up where necessary. You will ensure all medical appointments, tests, and lab work are completed as required. Additionally, you will provide 24-hour on-call phone coverage for emergency situations and assist with the AMAP certification process.

Responsibilities
* The assessment, planning, implementation, and supervision of medical care for individuals with developmental disabilities.
* Provide and supervise comprehensive nursing services and interventions to ensure the health care needs of individuals are met.
* Work with physicians as appropriate to address medical needs and follow through with provision of nursing care.

Requirements
* A Bachelor's degree in Nursing and licensure as a Registered Nurse in NYS.
* At least 1-2 years of experience working with individuals with developmental disabilities or mental health experience.
