Pediatric Nurse Coordinator

The Northwell Health is seeking a skilled Pediatric Nurse Coordinator to join their team in Bronx, NY. As a key member of the certified agency team, the Coordinator of Care will manage all aspects of patient care related to services provided in the home.

About the Role:

This role involves conducting home visits, completing required documentation, communicating with primary care physicians, and updating medication profiles. The Coordinator of Care will also coordinate care with other disciplines, review case communication notes, and participate in case conferencing with supervisors.

Qualifications:
  • Graduate of an approved school of professional nursing
  • Currently licensed RN in NYS
  • Home care experience preferred
  • Excellent verbal and written communication skills
Responsibilities:
  1. Perform a home care assessment to determine patient eligibility for services
  2. Develop and implement the Plan of Care based on a comprehensive physical, psycho-social, and environmental assessment
  3. Provide skilled nursing care as described in the 485-Plan of Treatment
  4. Orient and supervise home health aide personnel in accordance with regulatory requirements
  5. Evaluate the effectiveness of interventions in accordance with the plan of care
Additional Requirements:
  • Must be able to operate medical equipment
  • Must be able to perform physical and psychosocial assessments of clients and families
  • Must be able to travel
  • Occasional heavy physical exertion may be required

We estimate the salary range for this position to be between $95,000-$110,000 per year, depending on qualifications and experience.
