About the Job

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Mental Health Care Manager. As a member of our interdisciplinary team, you will play a vital role in coordinating care and ensuring the well-being of our residents.

Key Responsibilities

  • Coordinate and oversee the provision of nursing care, including medication management and wound care.\
  • Develop and implement individualized care plans, with input from interdisciplinary teams and residents and their families.\
  • Provide education and support to residents and their families on various health topics, including medication management and wellness strategies.\
  • Collaborate with community resources to ensure seamless transitions and continuity of care.\
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.\
  • Perform other related duties as assigned.\

Requirements

  • \
  • Current RN licensure in New York State.\
  • At least one year of post-RN licensure clinical nursing experience working with individuals with mental disabilities.\
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.\
  • Ability to develop and implement comprehensive care plans, with attention to cultural competence and client-centered care.\
  • Knowledge of professional nursing concepts, principles, and practices, including emergency treatments and interventions.\
  • Competitive salary range: $95,000 - $115,000 per year, depending on experience.\
