North Suffolk is looking for a Program Coordinator to join our Services over Sentences (SOS) Team The SOS coordinator will be the liaison to referring agency Suffolk County DA’S office to coordinate treatment plan and recovery coach assignment. This position will support the DA’s office on training projects to enhance the knowledge of Substance Use Disorder. This position will provide services that support entry into the recovery community and provide recovery support services to strengthen and enhance any clinical treatment services a person has or is receiving.

This a Full-Time position; 40 hours per week.

What you’ll do:

  • Coordinate with SOS assigned clinician in organizing, scheduling and brief interventions to engage referrals presenting for treatment
  • Implement, teach, assist and/or coach persons served with their individual action plan or recoverees who have developed their own wellness plan to review their goals and objectives on a consistent basis.
  • Liaison with families/guardians and interface with all collateral contacts working with the persons served or recoverees coached. (I.e. Probation Officers, individual therapists, and PCP).
  • Insures the program and individual records are current, comprehensive and meet agency and funding source standards.
  • Facilitates groups and develop and enhance peer driven educational services
  • Provide on-call (cell phone) coverage for the recoverees’ of the program. Respond to staff coverage, routine questions and system navigation

What you’ll need:

  • Education:
    • Bachelor's degree in a field other than behavioral health sciences or have a high school degree/GED required.
  • Experience:
    • Must have work experience with adults diagnosed with substance use and/or dependency.
    • Knowledge and experience of the addiction treatment service system, recovery coaching and other related human services.
  • Skills:
    • Certified Addictions Recovery Coach (CARC) certification preferred.
    • Valid driver's license and reliable transportation.
