institutions and in their communities. Project 180 is a leader in the forensic treatment space, offering
high-quality, specialized treatment programs that serve thousands of justice-involved individuals. Our
programs span criminal justice intercepts and include pre-arrest interventions, court-based diversions, jail in-reach, and intensive post-release services. Project 180 operates in partnership with County agencies including the Department of Mental Health (DMH), the Department of Health Services (DHS), and the Justice, Care and Opportunities Department (JCOD).

The Program
The Intensive Case Management Services (ICMS) program is funded by the Department of Health
Service (DHS) Office of Diversion and Reentry (ODR), the ODR/ICMS program provides intensive
wraparound services to the high-need/high-barrier behavioral health population in the LA County jail
system. The program provides permanent supportive housing for some of the most underserved or highest utilizers in the county system. Services are delivered through a recovery oriented, harm reduction framework.

The Position
Under the supervision of the Director of ICMS, the Program Coordinator is responsible for ensuring
effective organization, management and coordination of ICMS client care and data. A successful Program Coordinator will effectively collaborate with the ICMS team and external stakeholders to ensure effective program operations.

Essential Functions:
Coordinate client services and linkage
  • Manage new client referrals, including in custody and community referrals. Coordinate jail releases and initial interim housing placement. Assist the Program Manager in communicating caseload assignments to the treatment team.
  • Coordinate mental health services for clients. Schedule internal medication management appointments, provide linkage to external providers and complete associated administrative tasks.
  • Facilitate housing referral process with external stakeholders, housing providers and treatment team. Obtain ODR approval for transfers as needed, collect required paperwork and communicate housing placement, once obtained.
  • Schedule transportation services for clients and manage agency van reservations.
Maintain program files and ensure data quality
  • Create and maintain meticulous program and project trackers, completing data entry in program and P180 logs and databases.
  • Ensure data quality through review and analysis of internal and external QA reports, identifying missing data and indicators of program outcomes and engaging appropriate team members to resolve issues.
  • Manage ICMS client records. Maintain user-friendly, organized electronic files. Follow up to obtain missing documentation per program guidelines.
  • Identify and resolve operational issues preventing effective and efficient service delivery, escalating issues to the ICMS Director as needed.
Collaborate with a multidisciplinary team and external stakeholders
  • Liaise and maintain relationships with external stakeholders including but not limited to Department of Health Services/Office of Diversion and Reentry, Brilliant Corners, Housing for Health, LA County Sheriff’s Dept, interim housing sites and other service providers.
  • Actively contribute to planning and execution of program and administrative tasks in collaboration with the treatment team.
  • Provide responsive troubleshooting support throughout the workday for administrative and logistical issues faced by the Project 180 team.
  • Assist in the development and execution of special projects as identified by the ICMS Director
Fulfill baseline Project 180 staff expectations.
  • Regular attendance required. Holiday or weekend work may be required.
  • Maintain and uphold Agency mission statement, values, policies, procedures, and principles.
  • Perform other duties as needed.
  • Maintain active license and/or registrations as applicable (ie BBS, CCAPP)
Skills and Abilities
  • Organization: Able to manage and keep track of many processes, tasks, and items systematically, ensuring that nothing is overlooked or forgotten.
  • Process Design and Management: Skilled in developing and overseeing systems and processes to manage tasks in complex, highly-constrained environments, ensuring they meet the needs of various stakeholders.
  • Technical Proficiency: Fluent in MS Excel and/or Google Sheets. Quickly learns new systems and technology. Able to create and maintain data tracking systems.
  • Problem Solving: Capable of identifying issues, analyzing potential solutions, and implementing effective resolutions in a timely manner.
  • Attention to Detail: Focused on accuracy and thoroughness in completing tasks, carefully reviewing work to avoid errors and ensure high quality.
  • Communication: Skilled in clearly conveying information and ideas to diverse groups of people, both verbally and in writing; understands and meets customer/client needs and expectations.
  • Proactivity: Takes initiative to anticipate needs and potential problems, acting in advance to address them and continuously seeking ways to improve.
  • Analytical: Uses data to draw conclusions and identify next steps.
Minimum Qualifications
Required:
  • Valid CA driver’s license, proof of insurance and own vehicle required. Ability to transport clients in personal vehicle as needed.
  • Must not be on any type of legal supervision (probation or parole).
  • Verification of Employment Eligibility and Background Check
  • Strong computer skills. Fluent in MS Excel and/or Google Sheets
Preferred:
  • Experience working with the forensic and/or dual-diagnosed populations.
  • Experience working on an interdisciplinary team in a mental health or hospital setting.
  • Spanish-speaking
  • Bachelor’s degree in any field from an accredited college or university. In lieu of a Bachelor’s degree, at least 2 years of experience handling complex processes (e.g., scheduling, organization, etc) in a professional, office-based setting.

Environmental Conditions (Working Conditions)
Job duties are to be performed both in office and field settings, including Project 180’s project based
housing sites.

Physical Requirements
The employee is expected to move about the worksite throughout the day, which may involve walking,
standing, and climbing stairs, or using a mobility device, or similar. The employee is expected to drive,
carry a maximum of 20 lbs, listen, and speak in order to complete job requirements.

Mental Requirements
The employee must be able to do any/all of the following: perform complex and varied tasks such as
synthesizing information and making decisions without immediate supervision; handle routine
distractions, interruptions, noise, and uncontrollable changes in priorities/work schedules; interact
professionally and diplomatically in high-stress situations; create and follow plans; meet deadlines.
