JOB DESCRIPTION.

Family Youth Partners are responsible for assisting clients achieve their behavioral health goals by providing support services that include: supporting continuity of treatment, groups, transition between programs, and linkages to other community resources.

JOB COMPLEXITY.

  • Develop individualized client case plans
  • Access Flex Funds Procedures to ensure prompt responses to requests
  • Comply with confidentiality and privacy standards of protected health information and other applicable information and material in accordance to HIPAA and Harmonium policies and procedures

  • Provide individualized support/rehabilitation services to children, TAY population

  • Assist clients and families with identifying and managing obstacles to meeting treatment goals

MINIMUM COMPETENCIES.

  • Minimum of 4-year degree or combined equivalency of skills and demonstrated experience in the field of social services, human services, social work, counseling, case management, education, or the health services field

  • Experience as a consumer and/or family member of someone who was a recipient of any public, community, education, and/or social service agency

  • 2 years experience in Case Management, Family Support, Advocacy or related program preferred

  • Experience with Anasazi system and/or other electronic documentation systems

  • Bilingual-Preferred English plus: Spanish, Tagalog, Vietnamese or other 2nd language; specialty services require Bilingual Spanish/English


COMPENSATION

Starting Rate: $25.11 (all new hires begin at starting salary regardless of experience)

Full Time, up to 39 hours per week

Benefits: Includes medical, dental, vision, pet insurance, life insurance, long-term disability, critical illness, and a monthly $238.18 employer contribution. We also offer a 403b Retirement Plan with a match of up to 2% based on years of service. Time off accrual for this position includes 7 hours of sick time per month and 11.67 hours of vacation per month for the first 3 years and 14.58 hours per month thereafter.
