JOB DEFINITION
The Mobile Response Team (MRT) provides 24/7 mental and behavioral health services at Health through Housing (HtH) facilities and targeted Permanent Supportive Housing (PSH) sites across King County. The primary goal of the program is to provide in the moment crisis behavioral health services and time-limited behavioral health case management via mobile intensive response teams for the purpose of assisting individuals in becoming aware of behavioral health and other community service options. The MRT is a mobile interdisciplinary team consisting of certified peer counselors, behavioral health case managers, substance use disorder professionals, mental health professionals and Narcan waivered medical staff.
The case manager will provide services for up to 90 days to assist clients in re-connecting or connecting with care teams and community supports.
MAJOR DUTIES AND RESPONSIBILITIES
- Provide short-term, intensive case management focused on facilitating referrals to community resources and outpatient providers.
- Evaluate psychiatric, substance use, medical, and support needs for clients
- Assist clients with appointment reminders and coordinate transportation to assure best possible connection is made with outpatient providers
- Develop collaborative working relationships with housing programs and community support services
- Develop and maintain cooperative relationships with staff across DESC programs to ensure continuity of care between clinical and housing for clients
- Participate in psychiatric consultation, reflective supervision, shift handoff and team meetings, in-service trainings, and care conferences for clients.
- Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned.
- Maintain clean and healthy environment
- Maintain health, safety, and respect, for all staff and residents and consult with Clinical Supervisor and shelter staff regarding the equitable application of support and services
- Respond to emergent situations using Crisis Prevention Institute (CPI) de-escalation strategies
- Treat all clients and staff with dignity, respect, and positive regard.

Living Conditions
Participate in the planning, organizing and facilitating of unit mitigations for clients on your caseload. Unit mitigations are coordinated efforts to support clients with maintaining healthy living conditions. This can include but is not limited to attending care conferences related to unit mitigation, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with Housing staff to ensure proper work orders are filed in a timely manner, participating in cleaning out clients’ units, and documenting barriers to unit mitigation. *More specific details will be found in your program manual*
Requirements:MINIMUM QUALIFICATIONS:
- Ability to meet Washington Department of Health requirements for registration as a Registered Agency Affiliated Counselor (AAC) or any other superseding credential.
- Relevant Bachelor’s degree in social work, psychology, or related behavioral science, OR a combination of 1 year of relevant paid work experience and demonstration of the ability to perform required job duties.
- Ability to drive an agency or personal vehicle to conduct agency related business. A current Washington State driver's license and insurable driving record are required.
- Interest in working with clients who are difficult to engage and maintain in traditional mental health/substance use disorder programs.
- Interest and experience in working with adults experiencing homelessness, severe and persistent mental illness, and/or substance use disorders.
- Knowledge of and ability to assess mental health and substance use disorders, and familiarity with psychiatric medications
- Experience or interest in working with clients who are difficult to engage in traditional outpatient programs.
- Ability to remain open and receptive to constructive feedback.
- Bi-cultural or bilingual background preferred.
- Ability to work effectively with clients displaying a wide range of unpleasant and/or bizarre behavior.
- Ability to communicate and work effectively with staff from various backgrounds.
- Commitment to philosophy of harm reduction, housing first, and equity and social justice.
PREFERRED QUALIFICATIONS
Eligible for a Licensed AAC credential or any other superseding credential that meets RCW 71.05.020 requirements to act as a Mental Health Professional whose scope of practice includes independently conducting mental health assessments and making mental health diagnoses (Applicants who meet this qualification are eligible for a higher pay scale)
