PRIMARY PURPOSE 

The New Living Healthcare Services (NLHCS) Housing Community Support Worker (CSW) is responsible for providing housing advocacy and enrollment services to consumers under the direct supervision of the Clinical Director.  The NLHCS Housing Community Support Worker will provide professional-level service coordination for consumers who are at risk, or in danger of experiencing homelessness. The NLHCS Housing Community Support Worker will focus on assisting the consumers to achieve housing stability and self-sufficiency by implementing services based on each consumer's housing needs.  


The NLHCS Housing Community Support Worker position requires critical thinking skills combined with compassion and a sense of accountability for self and others.  In addition, the NLHCS Housing Community Support Workers must demonstrate cultural humility, respect, resilience, adaptability, conflict resolution and resourcefulness.  They also must have excellent interpersonal skills with the ability to build trusting relationships through collaboration which is vital to the engagement process. The NLHCS Housing Community Support workers will be under the direct supervision of the Clinical Director


The NLCHS Housing Community Support Worker must have the ability to multi-task.


MINIMUM QUALIFICATIONS 

  • High School Diploma or Equivalent 
  • 1-2 years direct case management experience working with at-risk and/or culturally diverse special needs populations
  • Medical documentation experience
  • Excellent analytical skills and attention to detail
  • Strong people skills and the ability to advocate for housing needs
  • Outstanding communication, presentation, and interpersonal skills 
  • Critical thinking and empathy
  • Working knowledge of Mental Health Core Service Agencies
  • Successful Federal Background Check

 

PREFERRED QUALIFICATIONS

  • A minimum of three years’ working with District of Columbia consumers challenged with mental health diagnosis;
  • Previous experience working in a Core Service Agency with a housing focus 
  • Three to five years’ experience working with homeless or vulnerable populations;
  • A minimum of three years of experience working within community systems, referring, case management;
  • Ability to maintain high productivity while functioning in a flexible environment;
  • Ability to apply crisis intervention and de-escalation skills;
  • Ability to apply theories of the Motivational Interviewing, Harm Reduction, and Trauma Informed Care


DUTIES AND RESPONSIBILITIES

  • Demonstrate New Living Healthcare Services Core Values.

  • Ensure the efficient and effective provision of housing services with dignity and respect in accordance with the agency’s mission.
  • Work with the NLHCS community support team as appropriate to ensure that the housing needs of the consumers are addressed.
  • Participate in weekly supervision and agency meetings, and professional development.
  • Work both independently and with the clinical team as needed. 
  • Travel throughout the District of Columbia to provide housing services as needed
    • Assess potential barriers to maintaining stable housing. Provide coaching, support and referrals to improve housing retention, stability, quality of life and self-sufficiency (e.g. providing support with: budgeting, effective communication skills, tenancy and life skills)
    • Facilitate introduction between participants and employment navigation agencies  to explore employment and training interests and opportunities.
    • Work with assigned consumers to assist in the creation of goal-based treatment plans and revise them when necessary. 
    • Provide coaching, community support and other services to help the consumers with making progress on their treatment plans including working with community collaborative partners. Maintain regular bi-weekly contacts.
    • Provide resources, referrals and follow-up for consumers to meet their needs and advocate on behalf of consumers to ensure they receive needed services.
    • Identify strategies to address any issues which have the potential to compromise achievement of goals and/or trigger relapse into homelessness.
  • Maintain encounter notes on consumers’ progress, complete all required housing tracking reports and data collection in a timely manner as deemed by the management at NLHCS.
  • Prepare for and participate in internal program audits.
  • Prepare requests for rental and utility assistance.
  • Contribute to the development and evaluation of the housing program
  • Participate in performance improvement and continuous quality improvement activities. 
  • Attend regular staff meetings and in-services. 
  • Complete annual education training requirements. 
  • Maintain regulatory requirements, including all state, federal and local regulations.
