About the Role

We are seeking a passionate individual to join our team as a Housing Stability Case Manager. As a key member of our team, you will play a vital role in supporting families in gaining and maintaining housing stability. Your primary responsibility will be to develop and implement case management plans that incorporate goals for both adults and children related to core areas such as finding stable housing, increasing income, and family stabilization and support.

Key Responsibilities
  • Meet face-to-face with families on caseload to pursue goal-related tasks, review progress, and address barriers to accessing services.
  • Collaborate with families and Housing Specialists to assist with housing search and placement process, including accompanying families to view units, advocating for families with landlords, and assisting families to successfully complete the application process.
  • Network with community resources for housing, employment, emotional wellness services, training, etc., and connect families as appropriate, including to COH health services.
  • Maintain regular communication in accordance with program contract guidelines with all families on caseload and document progressive engagement efforts with difficult-to-engage families.
  • Work consistently within a housing-first framework, providing clients with immediate access to available independent permanent housing and supportive services without prerequisites for sobriety or participation in psychiatric treatment.
Requirements
  • Associate's degree or two years of college.
  • In lieu of the education requirement, a minimum two years of experience with homeless services programs or provision of social services is required.
  • Bachelor's degree preferred.
  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license prior to starting.
  • Minimum of two years of experience working in housing/homelessness or social services field.
  • Strong knowledge of community resources or ability to become knowledgeable required.
  • Strong interpersonal and organization skills required.
  • Strong communication skills including verbal and writing.
  • Proven ability to submit timely, detailed, and accurate case notes.
  • Bilingual: Fluent in English and Spanish Preferred.
