Position Overview:

The Outreach Services Director oversees all operational aspects of Fourth Street Clinic's community outreach activities, including the mobile clinic, medical street outreach, community site services, and recuperative care programs. This role ensures that high-quality integrated care is provided to patients seen through outreach programs. The Outreach Services Director is part of and partners with the Senior Leadership Team to ensure continuity of care for patients, including access to behavioral health, dental, pharmacy, and other ancillary services. This leadership and supervisory position is responsible for modeling the organization's values and mission.

Core Responsibilities:

  • Oversee overall operations for all outreach programs, ensuring that the programs operate with high quality and minimal risk.
  • Ensure that outreach programs are integrated with all departments and services at the organization.
  • Develop and maintain partnerships with other homeless service organizations where outreach services are provided and needed.
  • Track and monitor data associated with program goals and effectiveness.
  • Oversee the outreach budgets and operational goals.
  • Adjust workflows and practices based on data to better meet the program objectives.
  • Ensure programs are compliant with grant funding requirements and work with the Grant Manager and Data Analyst to ensure data and narratives are accurate for grant reports and potential funding opportunities.
  • Collaborate with other clinic departments on the needs of the organization to ensure operational continuity.
  • Oversee vehicle maintenance schedules and licenses.
  • Maintain all policies and procedures for the operations of the outreach services, assuring compliance with Federal, State, and Local regulatory requirements.
  • Other duties as assigned by the COO.

Leadership Responsibilities:

  • Collaborate with the organization's Senior and Executive Leadership teams to ensure the delivery of high-quality, integrated care.
  • Lead out on organizational culture, supporting team members to find purpose and belonging in their work environment. Support the organization in its Strategic DEIB goals.
  • Supervise program managers and supervisors of the mobile clinic, street medicine, community sites, and recuperative care.
  • Represent Fourth Street Clinic at important partner and funder meetings, providing healthcare perspectives as part of community conversations.
  • Work to ensure that Outreach Services are viewed as the provider of choice in the community.
  • Function as an effective and responsive member of the SLVCEH Core function groups and associated Task Groups.

Required Qualifications:

  • A minimum of three years of progressive leadership experience within a community‐based organization.
  • Excellent communication skills, verbal and written.
  • Strong organizational skills and attention to detail.

Preferred Qualifications:

  • Degree in public administration, public health, or equivalent.
  • Prior experience in homeless services.
  • Prior experience working in medical outreach activities or medical clinics.
