Overview

The Housing Care Coordinator is responsible to the Housing Stabilization Navigator for providing housing-focused care management to neighbors who have transitioned from street homelessness to permanent housing. The end goal for the Housing Care Coordinator is to ensure that neighbors, once re-housed have all the tools and resources necessary to remain in permanent housing. The Care Coordinator’s role in the stabilization process begins after a neighbor has signed their lease and received initial interventions from the Housing Stabilization Navigator. Care Coordinators are expected to work with a careload of formerly homeless individuals, some of which will need more intensive services than others.

Responsibilities

Housing-focused Care Coordination

  • Maintain a careload of 15-25 individuals across Micah housing programs
  • Meet with participants in their homes, at least monthly and more often as appropriate, to assess needs specifically related to housing, noting other issues that could impede housing stabilization.
  • Build and maintain a positive rapport with individuals as a central component to their progress and treatment.
  • Screen and refer individuals for community resources, including medical, mental health, substance use, employment, disability, legal, life skills, and peer support services. A referral may include providing or setting up transportation for a neighbor to access the resource effectively. It may also mean accompanying them and/or setting up an advocate to attend a meeting with the participant. Resources may be external to the organization or internal (SOAR, PATH, Step Forward).
  • Follow-up on referrals, provide advocacy regarding individual progress and barriers and ensure successful connection.
  • Maintain a working knowledge of available local, state, and federal resources that provide support
  • Identify and coordinate crisis and re-entry care (i.e. emergency care, substance abuse treatment, adult services/in home care, returning home after incarceration or hospitalization, etc) as needed on behalf of neighbors.
  • Advocate with medical and insurance providers, mental health and crisis intervention teams, emergency services, probation and parole officers, and other service providers
  • Facilitate connections to natural supports, such as support groups, family reunification, faith-based organizations, friendships, etc.
  • Reassess and recalibrate Individualized Service Plans; monitor progress towards goals on a quarterly or annual basis, as applicable.

Documentation & Processing of Financial Assistance

  • Report appropriate financial assistance needs, including rental and utility subsides, by required deadlines.
  • Maintain neighbor files, schedules, and forms in a confidential and complete manner
  • Maintain documentation of neighbor encounters on at least a monthly basis, complete weekly reports on activities completed
  • Complete all quarterly and/or annual reporting requirements, as applicable

Team Participation

  • Participates in staff meetings and other group activities essential for the operation of an effective service.
  • Participates in team discussions regarding neighbor progress and lack of progress, with possible solutions to ensure best support for the neighbor’s success.
  • Promotes good community relations and utilizes community services and resources.
  • Attends scheduled training programs for professional development that includes, at a minimum, trainings required by Micah and by regulatory and accrediting bodies.
  • Assumes on-call responsibility as assigned.
  • Performs other duties as assigned.

Qualifications and Skills

Minimum Bachelor’s Degree in a human service with at least three to five years working with a special needs population (i.e. homeless, mentally ill, substance use, etc.) or commensurate experience or training. Demonstrated knowledge of homelessness, behavioral health, success planning, community rehabilitation and care management. Able to be flexible and appropriately handle stressful situations. Excellent communication skills, including the ability to network with landlords and develop relationships with community service providers. Must have an understanding of relationship-based service models and be comfortable working alongside volunteers. Must be a self-initiator and be able to problem solve various life issues.

Supervision:

The Housing Care Coordinator will be supervised by the Housing Stabilization Navigatoor. Performance evaluations will be completed by the Housing Stabilization Navigator on an annual basis.

Work Schedule:

This position is classified as a full-time exempt position. A flexible schedule will be necessary to perform job duties
