What is a Clinical Transformation Advisor:
The Clinical Transformation Advisory team utilizes the Lightbeam platform to optimize care management processes, improve clinical outcomes, and disseminate best practices to our client partners. This position is a client facing role and has direct reports. The CTA’s is primarily responsible for successfully implementing care management processes that are operationally sustainable, promote high-value care, and prepare organizations to succeed within their value-based contracts and other payer initiatives.
The ideal teammate will have successfully demonstrated clinical consulting experience with the ability to come up to speed on the population health industry and Lightbeam platform quickly. Expectations for this role include building and managing successful relationships with both new and existing clients, identifying their needs and goals to help them succeed using our population health management tools, and collaborating closely with internal stakeholders to help drive client and company success. Candidates should be energized by continuous personal improvement, proactively seeking out new skills and perspectives, exhibiting openness to coaching from senior colleagues, and contributing to the development of junior staff.
What You Will Be Doing:
Identifies development and educational opportunities for direct reports
Develop and implement strategic and operational plans across client contracts to drive successful outcomes
Identifies opportunities for growth of standard clinical transformation offerings
Identifies successes and challenges within the team and identifies areas of improvement
Pre-post management outcome reporting to demonstrate clinical program impact
Host client discussions pertaining to Care Management staffing model recommendations
Build and deployment of care plans and assessments within the Lightbeam platform
Conduct client training on the Care Management module
Analyze client operational workflows to identify opportunities to improve clinical efficiency, guideline adherence, and monitor outcomes of care
Decipher and relay actionable data insights in an intelligible way relating to client contracts
Ensure satisfaction, communication, and engagement with our customers
Build strong, credible working relationships across our client base
Identifies successes and challenges within the team and identifies areas of improvement
What You Will Bring:
Previous experience in a leadership role preferred
Experience in the healthcare industry, preferably with experience in value-based care and care management
Bachelor’s degree in health-related field with strong emphasis in clinical support, RN license preferred
Demonstrated knowledge of industry trends, issues, products, and solutions, as they relate to healthcare transformation and move to value-based arrangements
Ability to problem solve and regularly exercise initiative
Experience with developing executive level presentations
Ability and desire to work in a fast-paced, dynamic environment
Effective verbal and written communication
Analytical thinking and planning skills
Willingness and ability to travel (approximately 20%)
Comprehensive knowledge of all Microsoft Office applications (Word, Excel, PowerPoint, and Outlook)
