POSITION SUMMARY

Provides Service Facilitation and services to assigned individuals across the life span requesting and found eligible for the Comprehensive Community Services Program (CCS). Eligible Individuals served have a diagnosed mental illness and/or a substance abuse disorder.
QUALIFICATIONS

  • Works with adults and families to assess and develop goal oriented, time limited, strength-based service plans to address identified needs
  • Works to protect vulnerable adults and children, preserve families, or to keep out-of-home placement at minimum effective levels
  • Works to achieve active involvement and participation of individuals, family or significant others involved in the service program
  • Identifies, through case management and/or access services, individual needs which include medical, psychological, vocational, social, familial, and daily living spheres
  • Explains the consequences of non-compliance with court orders to adults and families who are involuntarily referred, monitors utilization of services, and informs the court of adherence to court orders
  • Facilitate adult and families' access to services, monitors cases to ensure that services are provided in an appropriate and timely manner, reviews progress, and terminates services when stated goals are reached
  • Teaches and advises individuals and families about available community resources and how to independently gain access to needed services
  • Works to achieve services and treatment objectives without duplication of effort and with maximum effective use of staff time and available resources
  • Maintains a complete record for all assigned cases; completes required written assessments, reports, and other related paperwork as necessary
  • Documents contacts and services, maintains files and data for reporting purposes, prepares court reports, collateral dictation, and prepares other records and reports
  • Attends and participates in staff meetings, supervision times, and other regularly scheduled meetings; develops professional knowledge and skills by attending training events, conferences, and workshops
  • Serves as advocate for clients within the community
  • Speaks to the community and other organizations to provide public education
  • Performs other duties as assigned

TRAINING AND PROFESSIONAL EXEPERIENCE QUALIFICATIONS

  • Bachelor's Degree in Social Work or a related field; and,
  • Minimum of 6 months of paid or unpaid experience in family, children, or adult case management services; or successful completion of an internship in the specific area of hire
  • Must have access to private transportation at all times for work-related duties.
  • Must be in compliance with State of Wisconsin Social Worker Certification requirements as a condition of employment by meeting one of the following requirements:
    1. Certified as a Social Worker in Wisconsin
      1. Possess or be able to obtain a temporary certificate within 2-months of hire and become certified within the specified time limits of the temporary certificate

DESIRED TRAINING AND PROFESSIONAL EXERIENCE QUALIFICATIONS

  • Paid or un-paid experience in a County Department of Human Services handling child protective services, adult services case management or service facilitation.
