Provides leadership in the coordination of oversight of Veterans receiving care to maintain or improve health, functional status and quality of life, as well as coping ability of the Veterans' caregivers in contract nursing home facilities.


Serves as an integral member of the Program Interdisciplinary Team (IDT) to address health needs, physical and cognitive function and need for social support for Veterans enrolled in the program using a holistic and individualized approach.

Ensures access to medically necessary outpatient serves as deemed appropriate and other appropriate medical specialty to patients in community nursing homes under VA contract.

Performs screening assessments to determine the needs of each Veteran.

Perform on-site follow-up and oversight visits to the Veteran after admission to the community nursing home to ensure that adequate and safe care is being provided while under VA contract.

Communication of treatment plan and goals to Veteran, caregiver and Providers as needed.
Effectively functions autonomously, as well as a member of the IDT, to ensure Veteran goals are met.

Serves as an advocate for the Veteran, ensuring access to health care and educating the patient and/or caregiver so that they are able to make well-informed health care decisions.

Collaborates and coordinates Veteran needs with Program leadership.

Coordinates care and provides required education of community nursing home staff to ensure adequate and safe care is being provided.
