Your Responsibilities:

  • Deliver direct nursing services to patients in non-hospital settings.
  • Required Skills: Proficient in lab draws, IV placement, and medication administration.
  • Engage patients in their healthcare journey, providing tailored educational support.
  • Establish and nurture meaningful relationships with patients and their families.
  • Implement a multidisciplinary care plan for patients enrolled in our Advanced Care program.
  • Collaborate closely with the Advanced Care leadership team to promote a redesigned care model that offers 24/7 comprehensive services in the home.
  • Oversee daily clinical operations, including planning and scheduling for patient care.
  • Ensure timely ordering and initiation of necessary durable medical equipment and services.
  • Act as the primary contact for all navigated patients, coordinating with medical, nursing, and ancillary services.
  • Work alongside medical providers and care staff to develop and execute patient education plans.
  • Monitor patient compliance with care plans and track progress throughout the care episode.
  • Assist patients and their caregivers in accessing community resources to address social determinants of health.
  • Evaluate patient progress towards goals during the post-care period.
  • Contribute to the development of educational materials and interdisciplinary care plans.
  • Perform other related duties as assigned.

Qualifications:

  • Current RN licensure in Virginia or a state participating in the multistate privilege to practice compact with Virginia.
  • A minimum of two years of experience in acute care nursing.
  • Current BLS and ACLS certifications.
  • Bachelor of Science in Nursing (BSN) preferred.

  • Clinical Proficiency: The Advanced Care RN must demonstrate strong clinical assessment abilities and effective prioritization skills, along with exceptional critical thinking to implement and evaluate patient care plans.
  • Effective Communication: Collaboration is essential; the Advanced Care RN must excel in teamwork and maintain open, responsive communication with colleagues and patients.
  • Cultural Sensitivity: The Advanced Care RN should be open to understanding diverse patient backgrounds and values, facilitating a culturally appropriate care plan.
  • Respect and Empathy: A commitment to patience and understanding is vital in establishing boundaries and expectations with patients.
  • Confidentiality Commitment: Adherence to patient confidentiality is paramount, requiring heightened awareness in navigation roles.

Physical Demands:

  • Ability to lift and carry equipment/supplies weighing up to 50 pounds over distances.
  • Capability to push or pull objects weighing up to 50 pounds.
  • Willingness to walk long distances and navigate multiple floors.
  • Frequent twisting, bending, and squatting throughout the workday.
  • Reaching and grasping with arms and hands, including overhead and below waist.
  • Ability to move within confined spaces and maintain balance in various positions.
  • Standing for extended periods during patient care.
  • Performing repetitive movements as required in patient treatment.

Additional Requirements:

  • Possession of a valid driver's license with a clean driving record.
  • Completion of required driver training upon hire.
