The RN Care Coordinator plays a vital role within a multidisciplinary team, collaborating with physicians, social workers, discharge planning assistants, and payers.
This role is dedicated to ensuring that patients receive quality-driven care throughout their acute episode and post-discharge, while maintaining efficiency, cost-effectiveness, and safety.
The position is focused on patient outcomes and is designed to foster a supportive environment.
Licensure and Certification Requirements:
Must possess a valid Registered Nurse license in the State of Virginia or be eligible for licensure.
Ongoing Requirements:
Maintain current RN licensure in Virginia and complete 15 Continuing Education Units annually.
Experience:
Required:
A minimum of three (3) years of nursing experience in an acute care environment.
Preferred:
Experience in academic healthcare settings, one (1) year of Utilization Management/Care Coordination experience, and clinical experience with specialized patient populations.
Education:
Required:
Baccalaureate Degree in Nursing from an accredited institution, or a Bachelor's degree in biological, health, or social sciences from an accredited program.
Preferred:
Master's Degree in Nursing or a related healthcare field from an accredited institution.
Key Responsibilities:
Collaborate with attending and consulting physicians to facilitate effective transitions throughout the hospitalization process.
Work in conjunction with all members of the multidisciplinary team to ensure that patient needs are met and that care delivery is coordinated across the continuum.
Engage with social workers to address psychosocial patient care challenges and develop comprehensive patient transition/discharge plans as necessary.
Interact with patients, families, healthcare professionals, and community agencies to ensure seamless care delivery.
Act as an advanced clinical resource for the team and serve as a liaison between the hospital and community agencies for the exchange of clinical and referral information.
Ensure compliance with Quality Improvement Organization (QIO) standards and CMS guidelines.
Utilize medical necessity criteria to assess the appropriateness of care levels and settings, assist in the denial and appeals process, and identify potential risk management issues.
Participate in and complete all patient safety initiatives relevant to the position.
Conduct all responsibilities in alignment with VCU Health's Mission and Values.
Establish and maintain professional relationships with providers, patients, families, and internal stakeholders.
