Position Overview: We are currently seeking a qualified MDS Coordinator to oversee the development and execution of personalized Care Plans for each resident within our Health Care Center. This role involves ensuring the accurate completion of the MDS in accordance with all relevant regulations.
Key Responsibilities:
- Develop and implement individualized Care Plans for residents.
- Accurately complete the MDS in compliance with applicable standards.
- Actively participate in the Interdisciplinary Care Team and attend resident care conferences.
- Share on-call responsibilities as part of the nursing administration team.
Qualifications:
- Must be a Registered Nurse with a degree from an accredited nursing program and possess a valid license in Virginia.
- Experience in a skilled nursing facility is preferred, ideally ranging from one to three years.
- Strong oral and written communication skills are essential.
- Proficiency in computer systems is required.
- Ability to effectively communicate with residents, families, and healthcare professionals.
- Excellent assessment skills and nursing judgment to create and update Care Plans.
- Familiarity with federal, state, and local regulations related to long-term care and the MDS process.
