Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at LifeLong Medical Care. As a Nurse Coordinator, you will play a vital role in coordinating team-based care, focusing on the intersection between medical and social needs of patients.

Key Responsibilities

  • Coordinate team-based care by identifying and addressing the medical and social needs of patients.
  • Act as a resource for support staff, providing guidance and support in triaging walk-in and telephonic care.
  • Facilitate chronic disease management activities, working closely with primary care providers to ensure complete evaluation of patient needs.
  • Coordinate primary care clinic services with other providers and agencies, ensuring seamless care transitions.
  • Collaborate with the Care Transitions team to ensure timely follow-up and return to primary care for hospitalized patients.
  • Utilize clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Coordinate care closely with other team members to identify patient self-care goals and ensure that medical and psychosocial needs are met.

Requirements

  • Bachelor's or associate degree in Nursing.
  • Current and valid California Registered Nurse license and BLS certification.
  • Proficient in Microsoft Office suite.
  • Knowledge of electronic health records (EHR), Epic preferred.

Preferred Qualifications

  • Minimum one-year experience in community health as an RN, but new graduates are welcome.
  • Knowledge of outpatient care, triage, and clinic flow.
  • Demonstrated ability and sensitivity to provide services to unhoused, undocumented, substance users, HIV (AIDS) infected, disabled, and psychologically impaired individuals.
  • Bilingual - Spanish strongly preferred.
