Join our team and make a positive impact on the lives of others We are looking for a Community Outreach & Intake Specialist II for our team in Oregon.

This is a hybrid role that requires a regularly scheduled commute of 2-3 days per week to our office in Portland, OR.

Position Overview:

This Community Outreach & Intake Specialist II position is a dynamic individual contributor role reporting to the Assistant Manager in Oregon. This unique position combines community outreach and Intake by reaching out to potential patients and caregivers as well as conducting home and virtual visits and orientations.

The Community Outreach & Intake Specialist II will provide outreach to connect to community centers, leaders, etc. to develop and maintain strong relationships and increase our patient enrollment. This position will be the face of FreedomCare in the targeted Oregon community and help grow the company through community partnership.

In addition, this specialist will be the a point of contact to speak with both potential and existing patients and caregivers and explain FreedomCare's offerings and care model, through demonstrating FreedomCare's core values of compassion, excellence, integrity, and respect, aligning with our mission to provide care to those who need it the most- the elderly and disabled.

Responsibilities:

Community Engagement (50%/day): as we acquire patients and home visits are scheduled, this time will lessen
  • Proactively create and nurture new relationships within Oregon communities by:
    • Scheduling and participating in daily outreach trips to connect with community members and potential referral sources (senior centers, hospitals, faith-based organizations, healthcare providers, etc.) providing information about FreedomCare's services and addressing any questions or concerns.
    • Organizing and hosting events to raise awareness and generate leads.
    • Developing and implementing tactics to effectively reach target demographics and communities.
  • Develop and maintain partnerships with local organizations, community leaders, etc. to expand outreach efforts; utilize industry contacts to enhance company visibility and increase our patients and caregiver enrollment.
  • Network with community leaders and professionals to build strategic partnerships.
  • Reporting: Independently create reports to track and monitor lead development. Track and report on outreach activities, including leads and partnerships established.
  • Effectively collaborate with the internal team to ensure alignment of outreach efforts with company goals.
  • Generate qualified leads for the OR program.
Intake Navigation/ Home Visits (50%/day):
  • Assist potential patients and caregivers with the intake process to get patients approved and on care.
  • Answer questions, gather information, and navigate any roadblocks that may arise during the enrollment process.
  • Ensure a quick and efficient turn-around time.
  • Provide a seamless customer service experience, while ensuring that our patients and caregivers receive the care they need.
  • Maintain regular communication with a variety of Case Management agencies regarding patient authorizations.
  • Conduct in-home assessments and virtual visits with patients and caregivers.
  • Facilitate a smooth onboarding process, introducing patients to their services with an in-home orientation.
  • Ensure clear and accurate communication with all parties involved.
  • Maintain meticulous records in our CRM system (Salesforce).
  • Navigate multiple systems and portals to determine eligibility and obtain authorization.
  • Maintain data input, scheduling, document uploading and initiate patient enrollment confirmation or rejection communications.
Experience & Qualifications:
  • Experience in speaking with different people influencing outcomes in an organized manner with the desire to build on existing skills and knowledge.
  • A reliable vehicle and valid driver's license.
  • Must have a proven track record building relationships quickly with a variety of stakeholders.
  • Must be able to communicate clearly to get a point across with the ability to highlight benefits of the situation.
  • Must have a background of taking initiative to complete tasks.
  • Strong organizational skills to keep track of assignments and be effective in both essential functions of the job including community liaison activity and intake work.
  • Time and priority management skills translating to the ability to handle several priorities at once without losing quality.
  • Proficient computer skills with a focus on CRM software (Salesforce a plus).
  • Ability to work independently while collaborating effectively with a team.
  • Strong follow-up skills to ensure tasks and projects progress and are completed in a timely manner.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $20.00 and $27.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
