Job Summary


Molina Healthcare Services (HCS) collaborates with members, providers, and multidisciplinary team members to evaluate, facilitate, plan, and coordinate an integrated delivery of care across the continuum, encompassing behavioral health and long-term care, for members with high need potential. HCS staff strive to ensure that patients progress towards desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.


KEY SKILLS

  • Acts as a community-based member advocate and resource, utilizing knowledge of the community and available resources to engage and assist vulnerable members in managing their healthcare needs.
  • Empowers members by assisting them in navigating and optimizing their health plan benefits.
  • Support may include scheduling appointments with providers, organizing transportation for healthcare visits, ensuring prescriptions are filled, and following up with members on missed appointments.
  • Aids members in accessing social services such as community-based resources for housing, food, employment, etc.
  • Performs outreach to locate and provide assistance for disconnected members with special needs.
  • Conducts research using available data to find members Molina Healthcare has been unable to reach through typical channels.
  • Engages in ongoing or project-based initiatives that may involve extensive member outreach via phone.
  • Assists members in maintaining Medicaid eligibility and accessing other financial resources as needed.

REQUIRED QUALIFICATIONS


EDUCATION:


High School Diploma/GED


EXPERIENCE:

  • Minimum 1 year of experience working with underserved or special needs populations, facing diverse health, economic, and educational circumstances.
  • Familiarity with Managed Care and knowledge of community resources, as well as understanding of HIPAA and confidentiality regulations.
  • Minimum of 2 years of experience working with members with behavioral health issues, psychiatric disorders, substance abuse, and physical ailments.

LICENSE/CERTIFICATION:


• Must possess a valid driver's license


PREFERRED QUALIFICATIONS


EDUCATION:


Associate's Degree in a healthcare-related field (e.g., nutrition, counseling, social work).


EXPERIENCE:

  • Bilingual proficiency based on community requirements.
  • Knowledge of healthcare systems beneficial.
  • Familiarity with community-specific culture.
  • Experience or understanding of health care fundamentals, community resources, social services, and health education.
  • Call Center Experience.
  • Experience in psychiatric unit or facility advantageous.

LICENSE/CERTIFICATION:


• Active and unrestricted Medical Assistant Certification
