Summary Description:

The Communities of Solutions (CoS) Coordinator plays a critical role as a program officer, subject matter expert, organizational/coalition coach, and internal advocate to support the development and implementation of community-driven health equity strategies through Prosperemos Juntos | Thriving Together (PJTT). This position works closely with other members of the CoS Team and an interdepartmental team to accompany coalitions in learning, failing forward, and ultimately addressing root causes of poor health.

Essential Duties:

  • Fulfill the role managing philanthropic interaction and support by developing trust with community coalitions, providing guidance, monitoring outputs and outcomes, and engaging other CoS team members and/or departments at MHM through trust-based philanthropy practices.
  • Make presentations in a variety of public settings on core PJTT frameworks, including Pathways to Population Health, stakeholder mapping, quantitative and qualitative measurement, driver diagrams, 90-day action planning, inclusion and engagement with persons with lived experience, and coalition governance. Continuously update knowledge and expertise on these and other topics as new research and practices in the field emerge.
  • Serve as a subject matter expert and advise coalitions addressing any of the Vital Conditions for Community Well-Being (thriving natural world, reliable transportation, humane housing, meaningful work and wealth, lifelong learning, basic needs for health and safety, belonging and civic muscle) based on the unique strengths and opportunities of their community
  • Assess, encourage, and provide guidance to coalitions as requested (at least once a month per coalition) to support the development and implementation of their community-driven health equity strategy.
  • Work with the MHM Community Connectors, persons with recent lived experience, and other PJTT stakeholders throughout MHM's service area in co-designing, co-creating, and co-evaluating PJTT and other CoS efforts through design, application review, check in, and other interdepartmental teams
  • Provide input, develop, and deploy capacity-building opportunities throughout MHM's service area in support of the Communities of Solutions framework; resources could include consultant contracts, trainings, leadership development, toolkits, webinars, and templates
  • Collaborate with MHM team members to advance and strengthen health equity as both an outcome and a process by integrating community residents and MHM team members with recent lived experience into decision-making processes
  • Collaborate with other members of the CoS Team to plan, organize, and carry out in-person and virtual events
  • Work closely with and provide support to Community Connectors in facilitating conversations, planning efforts, and group decision-making processes with communities through critical thinking, teamwork, collaborative problem-solving, and inclusion.
  • Facilitate community conversations with diverse audiences to share information and resources as well as collect information to support the planning, implementation, and evaluation of initiatives and programs
  • Identify, cultivate and maintain strong relationships and communication with a variety of external partners, including faith communities, community-based organizations, schools, and other nonprofits, ensuring that key stakeholders and MHM staff are kept appropriately informed and engaged, timely and consistently
  • Compile and analyze information to support assessment, evaluation, and reporting about the progress of community coalitions and PJTT more broadly towards health equity and impact goals, utilizing both quantitative and qualitative data.
  • Participate in the development of Communities of Solutions objectives and tactical plans
  • Represent MHM at community meetings and on community task forces as needed.
  • Perform other duties as assigned.
Salary:

Annual salary rate begins at $55,481.00. Mid range at $69,352.00. Actual starting rate will be commensurate with experience and education

Education and/or Experience:

High School diploma/GED required and a minimum of 5 years working in a community outreach or community organizing capacity required. In lieu of experience, a bachelor's degree in social/behavioral sciences, public health, or related field and 3 years working in a community outreach or community organizing capacity experience required. Advanced conceptual, analytical, change management, and organizational skills. Superior relationship development and influential skills and ability to engage both internal and external constituencies.

An understanding of community development models, especially Asset Based Community Development preferred.

Ability to manage multiple projects in a timely fashion. Excellent interpersonal skills, community building skills, public speaking skills, and conflict mediation skills.

This position requires the ability and experience to interact comfortably and professionally with MHM board members and team members including senior leaders, as well as with senior-level team members of other organizations across sectors including grassroots organizations as well as with community leaders and community members, and patients and clients.

Experience working with underserved populations and with specifically addressing disparities that impact health and wellbeing of individuals, families, and communities is required. Experience with the nonprofit sector and nonprofit business models preferred.

Strong verbal, organizational and written communication skills. Must be able to read, analyze, and interpret general business periodicals, professional journals, technical procedures or government regulations. Write reports, business correspondence, and procedure manuals. Ability to prepare presentations and documents that conform to prescribed style and format. Must be able to speak effectively and respond to questions before groups of customers, community members, leaders, or employees of an organization (diverse audiences). Must be fluent in English; Spanish bilingual is preferred.
