This role involves offering short-term case management, conducting assessments, and facilitating outreach efforts to promote stability, independence, and a safe living environment within our housing communities.

Key Responsibilities:

Provide direct support to residents through short-term case management and resource navigation.
Build and maintain a network of referrals for social services and community resources.
Manage and update databases for tracking resident needs, program participation, and communication.
Plan and coordinate workshops, social activities, and community events.
Conduct research and contribute to the development of special programs and resource initiatives.
Support the development of policies and procedures for community programs.
Identify and implement effective practices for case management and program operations.

Education and Experience:

1 or more years of college coursework in social services or a related field.
At least two (2) years of experience in social services, case management, or a related area.
Bilingual (English/Spanish)
