Key Responsibilities:

  • Oversee direct care staff to ensure compliance with care standards.
  • Provide training and support to agency staff regarding treatment orders and services in alignment with agency policies.
  • Manage referrals to ensure timely initiation of care or appropriate documentation for delays.

Conduct regular evaluations, set new objectives, and adjust care plans in collaboration with healthcare providers.

  • Complete all required supervisory and assessment visits in accordance with agency policies and regulations.
  • Facilitate coordination of care among direct care staff and external healthcare providers.
  • Act as a liaison between our agency and healthcare professionals regarding patient care.
  • Exemplify a positive attitude and professional conduct for direct care staff.
  • Provide orientation and competency assessments for new staff members.
  • Supervise newly hired clinicians in the field and ensure compliance with regulatory requirements.
  • Address and resolve patient care issues promptly.
  • Participate in staff training, continuing education, and quality improvement initiatives.
  • Engage in staff development activities and in-service meetings to enhance team knowledge.
  • Deliver training on state licensure requirements, clinical services, and agency policies.
  • Contribute to the administrative on-call team, maintaining accurate logs and follow-up documentation.
  • Assist in compiling data for agency evaluations and reports as assigned.

Qualifications:

  • Graduate of an accredited nursing program.
  • Experience in home health, pediatrics, or office settings is advantageous.
  • Bilingual candidates are preferred.
