We are offering a $1,000 sign-on bonus

We are in search of a full-time (35 hours per week) Activity Coordinator and Support Specialist at our Bridgeview Adult Day Health Center. The working hours for this role are 8:30 am - 3:30 pm, Monday through Friday (No weekends or major holidays).

Key Responsibilities:

  • Assist the Program Coordinator with assigned tasks.
  • Design and execute suitable activities tailored to the unique needs of each participant.
  • Support in the recruitment and training of volunteers.
  • Collaborate with the Program Coordinator to develop innovative ideas that enhance program growth and quality.
  • Maintain detailed records for each participant, including initial assessments, quarterly notes, and daily activity logs.
  • Engage in the bi-annual or quarterly review of each participant's care plan.
  • Contribute to the delivery of essential program services.
  • Create a monthly schedule of activities and events.
  • Adhere to reporting requirements set by the relevant regulatory agency.
  • Assist the Program Coordinator in implementing policies and procedures.
  • Monitor and report on ongoing mental and physical limitations of participants to the appropriate staff.
  • Provide basic nursing support, including:
    • Reporting changes in bodily functions.
    • Recording and obtaining vital signs.
    • Assisting with personal care and ensuring safe hygiene practices.
    • Supporting with Activities of Daily Living (ADLs) across various levels of functioning, including basic restorative nursing.
    • Recognizing and responding to emergencies, following established procedures, including CPR and the Heimlich Maneuver.
  • Encourage participants to engage in socialization, education, group activities, and recreational opportunities.
  • Assist with meal procurement, service, and cleanup, adhering to sanitary food preparation standards.
  • Occasionally provide transportation to community activities.
Qualifications:
  • A High School Diploma is required for the Activity Coordinator and Support Specialist position.
  • Certification as a therapeutic recreation specialist is preferred, or at least two years of experience in social or recreational programming within the last five years, including one year in a healthcare activities program.
  • If not certified, consultation with an occupational therapist is necessary to develop a therapeutic activities program tailored to participant needs.
  • A valid driver's license with a clean driving record is mandatory.

Successful completion of a MA State CORI check, CPS check, and company-paid pre-employment physical, drug and alcohol testing, as well as enrollment in our drug and alcohol testing consortium, is required.
