Key Responsibilities
Patient Care Coordination Specialist Duties:
- Oversee and support the daily operations of services provided to clients, ensuring compliance with state regulations and company policies while maintaining high-quality service coordination.
- Act as the primary support for daily electronic visit verification (EVV) and payroll operations.
- Manage and communicate authorization and scheduling modifications to guarantee service coordination and client satisfaction.
- Coordinate the deployment of Care Partner teams to meet fill rate expectations.
- Investigate and resolve payroll and billing inquiries under the guidance of the Client Service Manager.
- Serve as a liaison for Field Supervisors in communications with payer Case Management teams.
- Assist with the recruitment and onboarding processes for the team.
- Perform caregiver/Home Health Aide (HHA) responsibilities as required.
Qualifications
Patient Care Coordination Specialist Requirements:
- Prior experience in home health care or home care is highly desirable.
- A High School diploma or GED is required; an Associate's Degree is preferred.
- Experience as a caregiver is preferred.
- Staffing experience is a plus.
- A valid Driver's License is necessary.
