At Family Service Association, we are pleased to offer a $1,000 sign-on bonus.

We are currently in search of a full-time (35 hours per week) Activity Coordinator and Support Specialist for our Bridgeview Adult Day Health Center. The working hours for this role are from 8:30 am to 3:30 pm, Monday through Friday (No weekends or major holidays).

Key Responsibilities:

  • Support the Program Coordinator with assigned tasks.
  • Design and execute engaging activities tailored to the unique needs of each participant.
  • Assist in recruiting and training volunteers.
  • Collaborate with the Program Coordinator to generate innovative ideas for program enhancement and quality assurance.
  • Maintain comprehensive participant records, including initial assessments, quarterly notes, and daily logs.
  • Participate in the periodic review of each participant's care plan.
  • Contribute to the delivery of essential program services.
  • Create a monthly schedule of activities and events.
  • Ensure compliance with reporting requirements from relevant regulatory bodies.
  • Support the Program Coordinator in the application of policies and procedures.
  • Monitor and report on participants' ongoing mental and physical well-being to appropriate staff.
  • Assist with basic nursing tasks, which include:
    • Documenting any changes in bodily functions.
    • Recording vital signs.
    • Helping with personal care and hygiene.
    • Assisting with activities of daily living (ADLs) across various functional levels.
    • Recognizing and responding to emergencies, including administering CPR and the Heimlich Maneuver.
  • Encourage participants to engage in socialization, education, group activities, and recreational opportunities.
  • Assist with meal preparation, service, and cleanup, adhering to sanitary food handling and USDA standards.
  • Occasionally provide transportation for participants to community events.
Qualifications:
  • A High School Diploma is required for the Activity Coordinator and Support Specialist position.
  • Certification as a therapeutic recreation specialist is preferred, or at least two years of experience in social or recreational programming within the last five years, including one year in a healthcare activities program.
  • If not certified, collaboration with an occupational therapist is necessary to develop a therapeutic activities program that addresses participant needs.
  • A valid driver's license with a clean driving record is mandatory.

Applicants must successfully complete a MA State CORI check, CPS check, and company-sponsored pre-employment physical, drug and alcohol testing, and be enrolled in our drug and alcohol testing consortium.
