Acentra Health, a leading provider of healthcare solutions, is seeking a skilled Clinical Assessor to join our team. As a Clinical Assessor, you will play a critical role in ensuring that individuals receive the necessary care and support to maintain their independence and quality of life.
Key Responsibilities- Conduct Needs-Based Assessments: Evaluate the needs of individuals applying for Personal Care Services (PCS) and Community Alternatives (CAP) programs to determine their eligibility for these services.
- Develop Service Plans: Create personalized service plans that meet the unique needs of each individual, ensuring that they receive the necessary care and support to maintain their independence.
- Collaborate with Healthcare Professionals: Work closely with healthcare professionals, patients, and their families to ensure that individuals receive comprehensive care and support.
- Maintain Accurate Records: Ensure that all assessments, service plans, and other relevant documentation are accurate, complete, and up-to-date.
- Participate in Training and Development: Participate in ongoing training and development opportunities to enhance your skills and knowledge in areas such as person-centered thinking, planning, and awareness of the needs of persons with disabilities.
- Education and Experience: A Registered Nurse or LCSW license by the state of North Carolina, with 2+ years of experience in a home care setting.
- Knowledge and Skills: Knowledge of North Carolina Medicaid Clinical Coverage Policy (Clinical Policy) 3L and PCS Program Provider Manual, as well as standards of practice related to Medicaid-funded Personal Care Services, home and community-based services (HCBS) programs, and EPSDT.
- Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to interact effectively with individuals, families, and healthcare professionals.
