We are seeking an experienced Personal Care Assistant to join our team at Liberty Health. As a Personal Care Assistant, you will play a vital role in providing high-quality care to our residents, ensuring their comfort, dignity, and well-being.
Key Responsibilities- Assist residents with bathing, dressing, personal hygiene, and Activities of Daily Living (ADLs) as needed during shift.
- Change linen on beds with bath schedule and/or as directed.
- Take and record vital signs per policy and/or as directed.
- Assist Certified Nursing Assistants (CNAs) with needed tasks.
- Turn residents while in bed per Facility policy.
- Reposition residents while out of bed per Facility policy.
- Assist with oral hygiene, including denture care, when getting up in the morning, after meals, at bedtime, and/or when needed.
- Answer call bells promptly following Facility policy.
- Complete patient care records at end of shift, including accurate intake and output.
- Fill in Patient Observation Records and Activity Attendance Records for the next month at the end of each month.
- Collect urine or stool specimens as directed.
- Give enemas as directed.
- Keep patient unit/halls clean and neat.
- Keep bedside tables, chest of drawers, and closets clean, neat, and organized, including clothing.
- Care for and clean nursing equipment, including BP equipment and thermometers.
- Follow restraint policy, including release.
- Assist with post-mortem care when needed.
- Ensure residents have fresh ice water by refilling pitchers with ice and/or water at least once every shift.
- Clean water pitchers, cups, bedpans, and urinals per schedules.
- Rinse personal clothing, sheets, etc. if soiled before placing in appropriate laundry barrel.
- Assist residents with bedpan or urinal.
- Empty catheter bags at end of shift and record output on appropriate record.
- Assist with rehabilitation programs, such as bowel and bladder retraining, grooming, etc., and chart on Rehabilitation sheet every shift.
- Assist charge nurse with resident needs and treatments as directed.
- Keep a clean towel and a clean washcloth for each patient in bathrooms at all times, replacing immediately when used.
- Empty incontinent pad barrel and linen containers every shift and replace plastic bags in containers.
- Report at end of shift to oncoming CNAs.
- Understand and agree to follow policy of no transfers until Certified, and agree to obtain assistance from a CNA assigned to assist the Care assistant with tasks they are not comfortable or trained to perform and with all transfers.
- Must be 18 years of age or older and have a high school diploma or equivalent.
- Must be a graduate of a NA1 training program and have passed the program and must pass state test within 4 months from completion.
- Must be willing to care for geriatric patients and have a genuine concern for their welfare.
- Must be willing to receive, understand, and follow orders.
- Must be willing to work and cooperate well with other nursing staff, other departments, and families and visitors.
- Must be kind and respectful to patients, staff, and visitors.
- Must be able to use equipment without jeopardizing the safety of self, others, or equipment and use supplies safely and economically.
- Must wear approved colored dress uniform, or approved colored top and natural colored hose, white nonporous (leather) shoes, and nametag.
- Must demonstrate neat appearance and good personal hygiene.
- Must read, know, and follow personnel, department, and Facility policies and procedures.
- Must attend in-service training and other staff meetings as required.
