About This Role
Hartford HealthCare (HHC) is seeking an experienced Nurse Practitioner or Physician Assistant to join our REACH (Resource for Elderly And Care at Home) Program. As a key member of our geriatric care team, you will be responsible for providing high-quality patient care in the comfort of patients' homes in the greater Willimantic area.
Job Summary
We are looking for a motivated and compassionate healthcare professional to provide medical care to elderly patients in their homes. As a Nurse Practitioner or Physician Assistant, you will work closely with our multidisciplinary team to develop and implement individualized care plans that cater to the unique needs of each patient.
Responsibilities
- Perform routine physical examinations and formulate comprehensive medical plans to treat patients with complex geriatric conditions.
- Develop and implement treatment plans in collaboration with physicians and other healthcare professionals.
- Provide education to patients and their families on disease management and self-care techniques.
- Maintain accurate and timely records of patient interactions and treatment plans.
Requirements
- Degree: APRN (Family, Adult/Geri Nurse Practitioner) or PA Training
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Experience: At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
