Description

Work with Hartford Healthcare (HHC) to deliver exceptional patient care and drive quality outcomes.

As a Primary Care Advanced Practitioner, you will play a vital role in our Pre-Admission Center, providing preoperative testing for surgical patients at Windham and Backus Hospitals.

Key Responsibilities:

  • Coordinate preoperative care, including history and physicals (H&P's), and all required patient testing prior to procedures.
  • Consult with collaborating providers and specialists when necessary to achieve the best possible patient outcome.
  • Perform accurate preoperative physical exams in accordance with established licensure and certification.
  • Selectively order and interpret appropriate laboratory studies, X-rays, EKGs, and other diagnostic tests and procedures.
  • Participate in system-wide councils to standardize care provided by the Pre-Admission Center across the system.
  • Drive initiatives to improve surgical outcomes, reduce preventable readmissions, increase patient and provider satisfaction, improve patient outcomes, build relationships with community and hospital providers, and increase revenue to the organization.

Schedule

This is a full-time position, 40 hours per week, with (4) 10 hour shifts. No call, weekends, or holidays.

Qualifications

To be successful in this role, you should possess the following qualifications:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant Master's program.
  • Licensed as a Nurse Practitioner or Physician Assistant in the state of Connecticut.
  • Board Certification as an Adult Nurse Practitioner (ANP), Family Nurse Practitioner (FNP), Acute Care or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or certification as Physician Assistant.
  • A minimum of 1 year practicing clinically in-person as an advanced practitioner is required.

Estimated Salary: $120,000 - $150,000 per year, depending on experience and location.
