Geriatric Home Healthcare Provider

Hartford HealthCare (HHC) is seeking a full-time, experienced Nurse Practitioner or Physician Assistant for its REACH Program serving geriatric patients in the greater Willimantic area.
This role involves providing home-based care in a multi-disciplinary team setting. The ideal candidate will have at least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA).
We offer a competitive salary range of $110,000 - $140,000 per year, depending on experience, and a comprehensive benefits package including paid CME time off, reimbursed professional licenses and dues, and employee discount programs.

In this role, you will perform routine physical examinations, formulate medical plans, and provide appropriate treatment within the scope of your practice. You will also collaborate with physicians and care team members to develop plans for further evaluation and referral as needed.
To be successful in this position, you must have an unrestricted State of Connecticut Medical License (or eligibility) and be trained as an APRN (Family, Adult/Geri Nurse Practitioner) or PA. A minimum of two years of post-graduate experience is required.
