Job Summary

North Suffolk Mental Health Association is seeking a skilled Care Navigator to coordinate case navigation, participation, and transition for all CBHC programming. The successful candidate will educate and support individuals through the entire process, serving as a community liaison and dynamic team member who supports day-to-day operations of the programs.

Key Responsibilities
  • Case Navigation: Coordinate and perform referrals in a prompt, comprehensive, and responsive manner, including communication with referral sources, collateral providers, and community partners.
  • Client Support: Participate in outreach and engagement activities with program participants, including follow-up appointments, and provide support in triaging non-clinical calls.
  • Administrative Duties: Run client insurance in real-time, reconcile cases from call logs with state programs, complete bed searches for CCS, CBAT, detox, and other acute levels of care as needed, and maintain databases and spreadsheets for information tracking, communication, and client assignments.
  • Clinical Documentation: Complete clinical documentation as required in the electronic health record and/or other databases, and develop and implement individual action plans, monitor and report progress, and document as appropriate to credential level.
  • Team Collaboration: Participate in staff meetings and perform other duties and tasks as assigned.
Requirements
  • Education and Experience: Bachelor's Degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or related field of study, and a minimum of 2 years post-degree experience within a multidisciplinary treatment setting.
  • Skills and Qualifications: Group facilitation skills and experience with individuals with substance use disorders required.
