Job Summary

We are seeking a highly skilled and compassionate RN Care Manager to join our Street Team at Boston Health Care for the Homeless Program. As a key member of our multidisciplinary team, you will work closely with patients who experience chronic street homelessness, providing low-threshold, high-quality nursing and care management services in various non-traditional locations.

Key Responsibilities
  • Nursing triage and care management for patients in our Street Team Outpatient Clinic at MGH
  • Conduct street outreach and provide nursing care in this context as appropriate
  • Visit previously homeless patients in their homes and provide nursing care in this context as appropriate
  • Provide outreach-based care management and non-judgmental clinical support in collaboration with other Street Team members
  • Engage patients with substance use disorders and mental illness, with a strong commitment to harm reduction principles and trauma-informed care
  • Complete health/needs assessments and develop integrated care plans with the patient and primary care team
  • Track appointments, referrals, and primary care activities and follow up with patients to ensure they keep their appointments or reschedule if the appointment is missed
  • Be responsible for clinical quality improvement activities; document all encounters appropriately and help keep data platform updated
  • Participate in team meetings and case conferences
Requirements
  • Commonwealth of Massachusetts RN license. Minimum of 2 years nursing experience preferred
  • Understanding of substance use disorders and behavioral health issues
  • Care management experience preferred; interest and experience with underserved populations preferable
