Position Overview:

Compensation Range: $50/HR

The Clinical Transition Educator plays a vital role in delivering clinical education and training to facilitate the smooth transition of patients into the Option Care Health service model. This position also collaborates with the sales team to enhance the number of patients transitioning to our care model.

Key Responsibilities:

  • Assess, educate, and train patients, caregivers, and facility personnel on the integration of OCH services and products in alternative settings, at home, or through virtual platforms to ensure effective patient transitions.
  • Conduct thorough patient evaluations to ascertain their suitability for the OCH care delivery model.
  • Collaborate with the OCH sales team to identify educational needs related to clinical transitions and implement strategies to boost patient transition rates.
  • Uphold the confidentiality of patient and proprietary information while adhering to legal standards for safeguarding sensitive data.
  • Document educational activities accurately using OCH technology and applications.

Supervisory Role:

This position does not entail supervisory responsibilities.

Essential Qualifications:

  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.

Preferred Skills:

  • Proven ability to build and maintain relationships across various organizational levels and with external vendors.
  • Familiarity with industry standards and practices relevant to our services (e.g., ACHC, URAC standards).
  • Experience in delivering exceptional customer service to both internal and external stakeholders, ensuring quality service delivery and customer satisfaction.
  • Basic proficiency in PC applications, including internet, email, Microsoft Word, Excel, and PowerPoint.
  • Ability to identify operational challenges and propose effective solutions to enhance processes.
  • Access to reliable transportation for travel to care facilities, home visits, and multiple hospitals is essential. A valid driver's license and proof of insurance are required if using a personal vehicle.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational skills and the ability to deliver presentations effectively.

Travel Requirements:

This role requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide live educational support to patients, caregivers, and referral sources.

Preferred Experience:

  • Hands-on experience in home or alternate site infusion or discharge planning.
  • Experience in expanding partnerships with service providers.

Due to state pay transparency regulations, the salary range for this position is as follows:

Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, along with internal equity and market alignment.

Pay Range: $41.88 - $69.81
