Position Overview:
Hiring Range From: $50/HR
The Clinical Transition Educator plays a crucial role in delivering clinical education and training to facilitate the successful transition of patients to the Option Care Health care delivery model.
Key Responsibilities:
- Assess, instruct, and train patients, caregivers, and facility personnel on the integration of OCH services and products in alternative settings, at home, or through virtual platforms to ensure a seamless transition to the Option Care Health delivery model.
- Conduct thorough patient evaluations to determine their suitability for the OCH care delivery model.
- Collaborate with the OCH sales team to identify educational needs for clinical transitions and implement strategies to enhance patient transition rates.
- Safeguard the confidentiality of patient and proprietary information, adhering to legal standards for protecting sensitive data.
- Document educational activities accurately using OCH applications and technology.
Supervisory Responsibilities:
This position does not include supervisory responsibilities.
Basic Education and Experience Requirements:
- Active and unrestricted Registered Nurse (RN) license is required.
- A minimum of 2 years of experience in the healthcare sector is essential.
Essential Qualifications:
- Proven ability to build and maintain relationships with individuals at all organizational levels and within the business community.
- Knowledge of standard practices for all services offered, as well as current relevant standards (i.e., ACHC, URAC standards).
- Experience in delivering customer service to both internal and external clients, ensuring quality service standards and evaluating customer satisfaction.
- Basic computer skills: Proficient in using the internet, email, Microsoft Word, Excel, and PowerPoint.
- Experience in identifying operational challenges and proposing effective strategies for improvement.
- Access to reliable transportation for travel to care facilities, home visits, and various hospitals. A valid driver's license and proof of insurance are required if using a personal vehicle.
- Willingness to obtain nursing licensure in additional states as needed.
- Able to plan, organize, and deliver presentations effectively.
Travel Requirements:
This role requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide live education and training support to patients, caregivers, and referral sources.
Preferred Qualifications:
- Experience in home or alternate site infusion or discharge planning.
- Background in developing service provider partnerships.
Due to state pay transparency laws, the full range for the position is outlined below:
Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range is $41.88-$69.81.
