Position Overview:
Compensation Range: $50/HR
The Clinical Transition Educator plays a pivotal role in delivering clinical education and training to facilitate the seamless transition of patients into the Option Care Health care delivery model.
This position also collaborates with the sales team to enhance the volume of patients transitioning to OCH services.
Key Responsibilities:
- Assess, instruct, and train patients, caregivers, and facility personnel on the implementation of OCH services and products in alternative settings, ensuring a successful transition to the Option Care Health delivery model.
- Conduct thorough patient evaluations to ascertain their suitability for the OCH care delivery model.
- Collaborate with the OCH sales team to identify educational needs for clinical transitions and develop strategies to boost patient transition rates.
- Safeguard the confidentiality of patient and proprietary information while adhering to legal standards for privacy.
- Document educational activities accurately using OCH technology and applications.
Supervisory Role:
This position does not entail supervisory responsibilities.
Essential Qualifications:
- Active and unrestricted Registered Nurse (RN) license is mandatory.
- A minimum of two years of experience within the healthcare sector is required.
Preferred Skills:
- Proven ability to build and maintain relationships across various organizational levels and with external partners.
- Familiarity with standard practices and current applicable regulations (e.g., ACHC, URAC standards).
- Experience in delivering high-quality customer service to both internal and external stakeholders, with a focus on customer satisfaction.
- Basic proficiency in PC applications, including internet, email, Microsoft Word, Excel, and PowerPoint.
- Ability to identify operational challenges and propose effective solutions.
- Access to reliable transportation for travel to care facilities and patient homes, including a valid driver's license and insurance if using a personal vehicle.
- Willingness to obtain nursing licensure in additional states as needed.
- Strong organizational and presentation skills.
Travel Requirements:
This role requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide live education and training support.
Additional Qualifications:
- Experience in home or alternate site infusion and discharge planning is preferred.
- Proven track record in expanding service provider partnerships.
