Position Overview:
The Clinical Transition Educator plays a crucial role in delivering clinical education and training to facilitate the seamless transition of patients into the Option Care Health care delivery model.
Key Responsibilities:
- Assess, instruct, and train patients, caregivers, and facility personnel on the implementation of OCH services and products in alternative settings, at home, or through virtual platforms to ensure effective patient transitions.
- Conduct thorough patient evaluations to determine their suitability for the OCH care delivery model.
- Collaborate with the OCH sales team to identify educational needs related to clinical transitions and develop strategies to enhance patient transition rates.
- Safeguard the confidentiality of patient and proprietary information, adhering to legal standards for privacy.
- Document educational activities accurately using OCH technology and applications.
Supervisory Responsibilities:
This position does not include supervisory duties.
Basic Education and Experience Requirements:
- Active and unrestricted Registered Nurse (RN) license is required.
- A minimum of two years of experience in the healthcare sector is necessary.
Essential Qualifications:
- Proven ability to establish and maintain relationships across various organizational levels and with external vendors.
- Knowledge of standard practices for all services offered, as well as current relevant standards (e.g., ACHC, URAC).
- Experience in delivering customer service to both internal and external clients, ensuring quality service standards and evaluating customer satisfaction.
- Basic proficiency in PC skills, including internet, email, Microsoft Word, Excel, and PowerPoint.
- Ability to identify operational challenges and propose effective solutions to enhance processes.
- Access to reliable transportation for travel to care facilities, home visits, and various hospitals.
- Willingness to obtain nursing licensure in additional states as business needs arise.
- Strong organizational skills and the ability to deliver presentations effectively.
Travel Requirements:
This role requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide live education and training support to patients, caregivers, and referral sources.
Preferred Qualifications:
- Experience in home or alternate site infusion or discharge planning.
- Background in expanding service provider partnerships.
Compensation:
Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay range is $41.88-$69.81.
